Primary care nurses are critically important in delivery of day-to-day care of patients with coronary heart disease (CHD), diabetes and related cardiometabolic diseases.

The new Primary Care Nurses Network (PCNN) is developing innovative programmes which will help you to meet the goals established in the GMS contract and other standards of care.
