In CHD, diabetes and cardiometabolic diseases giving primary care nurses the opportunity to make their voices heard

Primary care nurses are critically important in delivery of day-to-day care of patients with coronary heart disease (CHD), diabetes and related cardiometabolic diseases.

The new Primary Care Nurses Network (PCNN) is developing innovative programmes which will help you to meet the goals established in the GMS contract and other standards of care.

Initially the PCNN will focus on:

• CHD
• Diabetes
• Cardiometabolic diseases, such as dyslipidaemia and obesity

Subsequent programmes will introduce a broader spectrum of diseases, including respiratory illness, allergy and women's health.
